Most side effects from common erectile dysfunction tablets such as sildenafil and tadalafil are mild and temporary. Headache, facial flushing, indigestion, nasal blockage and light-headedness are the symptoms patients report most often. Sildenafil can cause temporary colour-vision change in a small number of men; tadalafil is more likely to cause back or muscle ache. The symptoms that should not be “waited out” are collapse, severe chest pain, sudden major vision or hearing loss, or an erection lasting more than four hours. The most important preventable interaction is taking a PDE5 inhibitor with nitrate medicine for angina.

Which ED medicines are we talking about?

The most commonly used oral ED medicines are PDE5 inhibitors. This group includes sildenafil and tadalafil, along with vardenafil and avanafil in countries where they are available. They enhance the normal nitric-oxide pathway that allows penile blood vessels to relax during sexual stimulation.

Different medicines in this group share many side effects because they work through the same pathway, but their duration and enzyme selectivity differ.

Why do headaches and flushing happen?

These tablets support the nitric-oxide pathway throughout the circulation, not only inside the penis. The same vasodilation that helps penile blood flow can therefore cause headache, facial warmth/flushing and a small fall in blood pressure.

The effect is more noticeable in some men when combined with alcohol, dehydration, hot environments or other medicines that also lower blood pressure.

Why can tadalafil cause back pain?

Tadalafil lasts much longer than sildenafil and interacts with a slightly different set of phosphodiesterase enzymes. Back pain or muscle ache is therefore more characteristic of tadalafil. It is usually temporary, but troublesome pain may justify switching drug or treatment strategy after medical review.

What vision symptoms can occur?

Temporary colour-vision change or increased light sensitivity can occur with sildenafil in a small proportion of users. Mild transient symptoms are different from sudden loss of vision.

A brief colour tint is very different from sudden major loss of vision. Sudden visual loss is rare but urgent. Men with previous non-arteritic anterior ischaemic optic neuropathy or important eye disease should discuss suitability before using a PDE5 inhibitor.

Can ED medicines cause low blood pressure?

A small fall in blood pressure is expected. For most otherwise stable men this is not dangerous, even when standard blood-pressure medicines are being used. Problems arise when the baseline pressure is low, several vasodilators are combined, dehydration is present, or nitrate medicines are taken.

Symptoms such as fainting, near-fainting, marked weakness or chest discomfort need immediate assessment.

The nitrate interaction: the most important safety rule

The nitrate interaction is the safety rule every patient should know. PDE5 inhibitors must not be combined with organic nitrates used for angina, recreational nitrate “poppers”, or nicorandil. The combination can produce an unpredictable and potentially dangerous fall in blood pressure.

If you develop chest pain after taking an ED tablet, tell the emergency team exactly what you took and when. Do not take a nitrate on your own.

Priapism: when an erection becomes an emergency

Priapism means a prolonged erection that does not settle normally. It is more classically associated with penile injection treatments than with oral PDE5 inhibitors, but it can occur.

An erection lasting more than four hours requires urgent emergency/urology care because prolonged ischaemia can damage penile tissue. Do not wait overnight hoping it will disappear.

What about penile injections?

Intracavernosal injections such as alprostadil can be highly effective when tablets fail or cannot be used. Their important side effects include penile pain, bruising, prolonged erection/priapism and, with repeated use, fibrosis or plaque formation. They should be started with proper office training rather than purchased and injected without instruction.

What about herbal or “natural sex” tablets?

Unregulated sexual-enhancement products are a separate safety problem. Some may contain undeclared PDE5 inhibitor-like drugs or multiple stimulants. This is particularly dangerous in patients using nitrates or heart medicines because neither the ingredient nor dose may be known.

“Herbal” is a marketing description, not a safety guarantee. If the ingredient and dose are uncertain, the interaction risk is also uncertain.

When should you stop the medicine and seek urgent care?

  • Chest pain, severe breathlessness or collapse.
  • Fainting or persistent very low blood pressure symptoms.
  • An erection lasting more than four hours.
  • Sudden major loss of vision.
  • Sudden hearing loss, especially with dizziness or ringing in the ear.
  • Severe allergic swelling or breathing difficulty.

If side effects are mild, what can be changed safely?

  1. Use a licensed medicine prescribed after reviewing your health and other drugs.
  2. Do not mix different ED tablets or add extra doses on your own.
  3. Avoid heavy alcohol intake around treatment.
  4. Review nitrate, nicorandil and alpha-blocker use before starting.
  5. If one PDE5 inhibitor causes troublesome effects, discuss another option rather than abandoning ED treatment entirely.
  6. Treat the underlying ED cause so that medication is not the only strategy.

What to bring for consultation

  • All medicines and supplements, including occasional heart tablets.
  • Details of the ED medicine, timing and side effect experienced.
  • Blood pressure readings if dizziness occurred.
  • Heart disease history and any nitrate/nicorandil use.
  • Kidney/liver reports if you have known organ disease.
  • Photos of the medicine strip or bottle if purchased outside a regular pharmacy.

Are sildenafil side effects dangerous?

Most are mild and temporary, but nitrate interactions, severe hypotension, priapism and sudden vision/hearing symptoms need urgent attention.

Why does tadalafil give me back pain?

Back or muscle ache is a recognised tadalafil effect and is usually temporary. Persistent or severe pain should be reviewed.

Can I take a painkiller for the headache?

That depends on your medical history, kidney function and other medicines. Recurrent headache is better managed by reviewing the ED medicine rather than repeatedly self-treating it.

Do ED tablets damage the heart?

In appropriately selected men, PDE5 inhibitors have a good cardiovascular safety profile. The main issue is whether sexual activity is safe and whether nitrates or other contraindicated medicines are being used.

What if I get chest pain after an ED tablet?

Seek urgent medical care and tell the team which ED medicine you took and when. Do not self-administer a nitrate.
