Risks of Testosterone Therapy
Testosterone therapy can be useful for men with confirmed hypogonadism, but it is not risk-free. The most important practical risks include a rise in haematocrit (too many red blood cells), suppression of sperm production, acne or oily skin, breast symptoms, fluid retention and formulation-specific side effects. Prostate and cardiovascular safety need individual assessment rather than blanket reassurance or fear. Safe TRT starts with the correct diagnosis, discussion of fertility plans and baseline tests, followed by regular monitoring and dose adjustment.
Why the risks of testosterone therapy depend on the patient
TRT risk is influenced by age, baseline haematocrit, sleep apnoea, smoking, cardiovascular disease, prostate history, fertility plans, dose and formulation. Many complications are avoidable when treatment is prescribed for a clear indication and monitored rather than used casually.
Common and important side effects

A baseline CBC and repeat haematocrit after treatment starts are important. If it rises substantially, the clinician may need to reduce the dose, change formulation, pause treatment or investigate contributing causes.
Testosterone therapy can suppress fertility
TRT can markedly reduce or stop sperm production. A man may still have normal erections and ejaculation while his sperm count falls. This is why fertility plans must be discussed before treatment.
- Do not use standard exogenous testosterone while actively trying to conceive.
- If future fertility matters, consider semen analysis or sperm banking in selected situations before long-term therapy.
- If testosterone has already been used, recovery of sperm production may take months and sometimes longer.
Does testosterone therapy cause prostate cancer?
Current evidence does not support the old idea that correctly prescribed TRT simply “causes” prostate cancer in every man. At the same time, testosterone is biologically relevant to prostate tissue, and men with known or suspected prostate cancer require specialist assessment.
Baseline prostate risk should be considered before TRT when age and circumstances make PSA screening appropriate. A significant PSA rise during treatment should be evaluated rather than automatically blamed on testosterone or ignored.
Cardiovascular and clotting concerns
The cardiovascular safety discussion has evolved as better trials have become available, but individual risk still matters. TRT should not be used as a cardiovascular prevention treatment. Men with recent major cardiovascular events, uncontrolled heart failure, thrombophilia or substantial risk factors need careful medical review before starting.
A rise in haematocrit is a separate safety issue that can increase concern about thrombosis and is one reason regular blood monitoring matters.
Sleep apnoea, urinary symptoms and other cautions
- Untreated severe obstructive sleep apnoea should be addressed before or during specialist consideration of TRT.
- Significant lower urinary tract symptoms should be evaluated; do not assume every urinary change is caused by testosterone.
- Liver toxicity is not a general feature of all modern testosterone formulations, but unregulated oral anabolic products can be very different from prescribed TRT.
- Mood or libido changes can occur if testosterone levels swing too high or low; dose and formulation may need adjustment.
Risk is not the same for every formulation or every patient
A 28-year-old man who wants children, a 58-year-old man with a high baseline haematocrit and a prostate-cancer survivor do not have the same TRT risk profile. Some injectable regimens can produce larger testosterone peaks and are more prone to raising haematocrit than steadier transdermal delivery; the exact preparation, dose and follow-up therefore matter.
Cardiovascular safety also deserves a balanced discussion. TRT should not be prescribed to prevent heart disease, but appropriately indicated treatment is not automatically a cardiovascular hazard in every man. Recent major cardiovascular events, uncontrolled heart failure, thrombophilia and important baseline risk still need individual assessment before treatment.
How safe TRT is monitored
- Confirm diagnosis with symptoms plus repeat low testosterone.
- Check baseline haematocrit and relevant prostate risk.
- Measure testosterone at a formulation-appropriate time.
- Repeat haematocrit after starting and periodically thereafter.
- Review symptom benefit; continuing treatment without benefit should be questioned.
- Review fertility plans at follow-up because life plans change.
Symptoms that should not wait for routine TRT follow-up
- You develop chest pain, sudden breathlessness, weakness of one side or another possible vascular emergency.
- A leg becomes acutely painful and swollen.
- You cannot pass urine.
- You have a severe allergic or injection reaction.
Before a TRT safety review
Bring these if available:
- Original testosterone reports and dates/times of sampling.
- Current TRT prescription, formulation, dose schedule and date of last dose.
- CBC/haematocrit results before and after treatment.
- PSA results if relevant.
- List of other medicines, supplements and anabolic agents.
