A penile implant is designed for long-term use, but it is not guaranteed to last for life. Modern inflatable prostheses commonly function for 10 years or longer, and many continue well beyond that. In a long-term meta-analysis, estimated device survival was about 87% at 5 years, 77% at 10 years and 64% at 15 years. Individual results vary with device type, surgical history, infection, mechanical wear and how revision is defined. A device that is comfortable and working normally does not need routine replacement simply because it reaches a certain age.

These are pooled estimates across different devices, eras and definitions of failure. They are useful for counselling but cannot predict exactly when an individual implant will need revision. Contemporary device design has continued to evolve.

What usually causes an implant to stop working?

  • Mechanical wear of the pump, cylinders, tubing or reservoir in an inflatable system.
  • Fluid leak or loss of pressure so the implant no longer becomes fully rigid.
  • A pump or valve that becomes difficult to operate.
  • Device malposition or cylinder problems.
  • Infection or erosion, which is not a simple mechanical failure and requires urgent specialist assessment.

Do malleable implants last longer?

Malleable implants have fewer moving parts, so there are fewer opportunities for pump, valve or fluid-system failure. They can still develop complications such as erosion, pain, migration or infection. Durability is only one factor when choosing between malleable and inflatable devices; concealability and ease of use are equally important for many patients.

Signs that an inflatable implant may have a mechanical problem

  • The pump suddenly feels different or will not transfer fluid.
  • The penis no longer becomes as firm as it previously did despite correct pumping.
  • The device inflates but loses rigidity quickly.
  • You cannot deflate a device that previously deflated normally.
  • One side looks or feels markedly different from before.

What happens if it eventually fails?

A urologist first confirms whether the problem is device mechanics, technique, positioning, pain, infection or another issue. If there is true mechanical failure, revision surgery can replace the affected components or the whole system depending on device age, model, findings and surgeon judgement. Revision surgery can be more complex than a first implant because of scar tissue and carries a different infection-risk profile.

Should a working implant be replaced preventively?

Usually no. There is no fixed expiration date after which a comfortable, functioning prosthesis must be changed. Preventive replacement exposes a patient to another operation without a clear benefit. Continue routine clinical follow-up as advised and seek review if function changes, pain develops or there are signs of infection or erosion.

Device survival is not the same as a guaranteed lifespan

Long-term studies describe the proportion of implants still functioning at different time points; they cannot predict the exact year an individual device will fail. In a 2022 meta-analysis of inflatable prostheses, estimated device survival was about 87% at 5 years and 77% at 10 years, with some devices functioning far longer. A younger man should therefore enter surgery understanding that revision may eventually be needed even when the first operation and early recovery are completely successful.

Age changes the reliability discussion

For a man in his 40s or 50s, even excellent 10-year device survival still means that one or more revisions may be needed over a lifetime. For an older man, the same survival curve may have a different practical meaning. This is why implant longevity should be discussed as lifetime revision probability rather than as a promise that a device will last a fixed number of years.

What to ask about long-term device reliability

  • Which device is being proposed and what its manufacturer warranty covers in your setting.
  • What mechanical problems most commonly lead to revision for that device type.
  • Whether your age makes future revision during your lifetime reasonably likely.
  • What symptoms should trigger review even if the device still partly functions.
  • How revision differs after a clean mechanical failure versus infection, erosion or severe corporal fibrosis.

FAQs

Can a penile implant last 20 years?

Yes, some do. Long-term pooled data show that a meaningful proportion of inflatable implants remain functional at 15–20 years, but individual longevity cannot be guaranteed.

Will frequent sex wear the implant out faster?

These devices are designed for repeated cycling and sexual use. Normal use is expected; there is no good reason to avoid sex simply to preserve a working prosthesis.

Does an implant need servicing?

No routine servicing is required inside the body. Follow-up is clinical, and revision is considered only if there is a problem.

Is revision always a full replacement?

Not always. The decision depends on which component failed, device age, infection risk and intraoperative findings. Some surgeons prefer replacing the entire system in certain situations.
