Normal Penis Size: Medical Explanation
There is a wide normal range of penis sizes. The best-known systematic review of clinician-measured data found an average erect length of about 13.1 cm and average erect circumference of about 11.7 cm. Average stretched flaccid length was about 13.2 cm. These are population averages, not targets that every man must reach. Measurement technique, body fat, temperature and erection quality can change the number. A true micropenis is diagnosed using standardized stretched penile length relative to age/population norms, not from comparison with pornography, photographs or locker-room impressions.
What studies show about average penis size
The review included up to 15,521 men across the different measurements, but not every participant had every measurement recorded. Erect measurements had smaller sample sizes than stretched measurements. The numbers therefore provide useful reference points, not an absolute definition of normal for every population.

How doctors measure penile length
For medical assessment, a standardized stretched penile length is useful. The EAU describes measurement from the pubic region to the glans using a consistent technique, often compressing the suprapubic fat pad when measuring bone-to-tip.
- Measure along the top (dorsal) surface.
- Compress the fat pad to the pubic bone when using bone-to-glans measurement.
- Measure to the tip of the glans, not the foreskin.
- Use a fully stretched but not painfully pulled flaccid penis for stretched length.
- For erect length, use the same anatomical reference point.
Why flaccid size is especially misleading
Flaccid length can change substantially with temperature, anxiety, exercise and sympathetic tone. Some men have a smaller flaccid penis that expands more during erection; others change less. This is why casual flaccid comparison is a poor way to judge erectile size.
What is a micropenis?
Micropenis is a medical diagnosis, usually congenital or endocrine in origin, in which stretched penile length is approximately 2.5 standard deviations below the mean for the relevant age and population. It is not simply “below average”. Adult men who think they have a micropenis often have a normal measurement when assessed objectively.
Normal size does not guarantee confidence, and larger size does not guarantee sexual satisfaction
Sexual function depends on erection quality, arousal, communication, stimulation, comfort and relationship factors. Anxiety about size can itself reduce erection quality and enjoyment. For many couples, focusing on erection quality and sexual communication is more useful than chasing a centimetre target.
What can make a normal penis look shorter?
- Suprapubic obesity or a buried penis.
- Viewing angle from above.
- Incomplete erection.
- Cold temperature or anxiety in the flaccid state.
- Peyronie’s disease or scarring causing acquired shortening.
- After some pelvic surgeries.
Average is not the same as a minimum normal size
An average erect length around 13.1 cm is a population mean, not a pass/fail threshold. Many healthy men are below that mean and many are above it. Using the average as the definition of “normal” would incorrectly label a large proportion of healthy men as abnormal.
Micropenis is different: it is defined statistically using stretched penile length relative to appropriate age and population norms, usually around 2.5 standard deviations below the mean. It cannot be diagnosed because a man is simply below average or because a flaccid penis looks small.
Penis enlargement: important safety points
There is no tablet or supplement proven to permanently lengthen a normal adult penis. Devices, fillers and surgery have specific risks and should not be used simply because online comparisons create anxiety. EAU guidance recommends screening for body dysmorphic concerns and strongly advises against injection of silicone, paraffin or petroleum jelly.

- Concern for true micropenis.
- New loss of length or new curvature.
- Painful erections or penile plaque.
- Buried penis causing urinary, hygiene or sexual problems.
- Considering cosmetic penile injections or surgery.
- Severe size anxiety despite normal measurements.
When size is a medical issue, not just a measurement question
- Penile injury with sudden swelling/bruising.
- Severe infection or skin breakdown after an injection/filler.
- Erection lasting longer than four hours.

- Any prior standardized measurement if available.
- History of puberty, hormonal disorders or genital surgery if relevant.
- Weight and buried-penis symptoms.
- Any new curvature, pain, plaque or shortening.
