PSMA PET Scan for Prostate Cancer Explained
A PSMA PET scan is a highly sensitive imaging test used in selected patients with prostate cancer to look for disease in lymph nodes, bones or other organs. PSMA (prostate-specific membrane antigen) is a protein commonly expressed on prostate-cancer cells; a radioactive tracer binds to it and is detected by PET imaging, usually combined with CT. PSMA PET is useful for initial staging of higher-risk disease and for locating recurrence when PSA rises after treatment. It can detect small deposits missed by conventional scans, but it is not perfect: some prostate cancers show little PSMA uptake and benign tissues can also take up tracer.
When is PSMA PET used?
- Initial staging of selected unfavourable intermediate-, high- or very-high-risk prostate cancer
- Biochemical recurrence after radical prostatectomy or radiotherapy
- Clarifying suspicious findings on conventional imaging
- Planning metastasis-directed treatment or systemic therapy in selected recurrent disease
How the scan is performed
A small amount of PSMA-targeted radiotracer is injected into a vein. After an uptake period, images are acquired on a PET/CT scanner. The test itself is not surgery and usually does not require anaesthesia. Preparation instructions vary with tracer and centre.
What does “PSMA-avid” mean?
It means a focus has taken up the tracer more strongly than expected. The pattern, CT anatomy and known physiological uptake are considered before calling it metastasis. Salivary glands, kidneys, bowel and other normal tissues can show uptake.
Can PSMA PET miss cancer?
Yes. Microscopic disease may be below PET resolution, and a minority of aggressive prostate cancers have low or heterogeneous PSMA expression. A negative scan therefore cannot guarantee that cancer is confined to the prostate.
How PSMA PET can change treatment
Finding unsuspected nodal or distant disease may shift a patient away from prostate-only treatment or change radiation fields/systemic therapy. Conversely, limited recurrence may allow targeted salvage strategies. Treatment should not be based on one bright spot without appropriate expert interpretation.
A negative PSMA PET is not a guarantee of microscopic clearance
Detection falls at very low PSA levels and some prostate cancers express little PSMA. After prostatectomy, a negative scan should therefore not automatically delay potentially curative early salvage radiotherapy when the PSA pattern and pathology support treatment.
A positive scan should answer a management question
PSMA PET is most useful when the result could alter radiation fields, identify oligometastatic disease, clarify equivocal conventional imaging or establish more extensive metastatic spread. Ordering it simply because it is technologically advanced can add cost and incidental findings without changing treatment.
Not every PSMA-avid focus is prostate cancer
Benign ganglia, fractures, inflammatory lesions and some non-prostate tumours can show uptake. Experienced nuclear-medicine interpretation, CT correlation and the pattern of spread are important before a single focus is labelled metastatic disease.
When to seek earlier medical review
Contact the treating team when a PSMA PET finding would change curative treatment, especially if it is isolated or unexpected. A lesion that is atypical for prostate cancer may need correlation or biopsy before abandoning a potentially curative plan.

FAQs
At what PSA will PSMA PET detect recurrence?
Detection probability generally rises as PSA rises, but useful scans can be positive at low PSA values. The decision to scan also depends on PSA kinetics and planned salvage treatment.
Is PSMA PET better than a bone scan?
For many staging and recurrence settings it is more sensitive, but the exact recommended imaging pathway depends on disease risk, availability and treatment plan.
Does PSMA PET require stopping prostate medicines?
Usually not automatically. Follow the nuclear-medicine centre instructions and tell them all current therapies.
Can PSMA PET confirm prostate cancer without biopsy?
It can strongly support staging, but primary diagnosis and Grade Group generally require tissue unless there is an exceptional clinical scenario.
